For Manchester City, the mental effect is brutal. They must score four goals without reply, or three and hope for extra time. That kind of comeback in the Champions League has been seen before, but it doesn’t happen often.

This fixture has become one of the most recurrent rivalries in modern European football. It is the fifth consecutive season in which these two clubs have met in the knockout rounds.

Manchester City eliminated Madrid in 2023, before the Spanish side took revenge in 2024 on penalties, then again in 2025 courtesy of a brilliant Kylian Mbappé display at the Bernabéu.

Match Preview: Manchester City vs Real Madrid

Manchester City have accrued 61 points from 30 Premier League matches, second behind Arsenal by nine points. The title race is, in practical terms, over, even if Guardiola will never admit to such publicly.

A 1-1 draw at West Ham on Saturday exposed a side struggling to convert opportunities. Erling Haaland has scored only four times in his last 17 appearances across all competitions.

Guardiola acknowledged the gruelling schedule that led him to give his players a rest day, saying;

“In Madrid, we arrived late [back] in the hotel, sleep, come back in the morning, travelling here, day after no training, go to London, arrived [back] yesterday at 2:30am,” the former Barcelona coach said in quotes revealed by 101 Great goals.

The City squad trained only last night, an unusual decision ahead of such a high-stakes European night.

The positive news for the Cityzens is that Guardiola can focus exclusively on the Champions League this week.

The Etihad will be at full voice, the atmosphere will be electric, and absences on the City side are limited: Joško Gvardiol (tibial fracture) and Rico Lewis (ankle) remain sidelined, but the remainder of the squad is available.

Erling Haaland was rested during the FA Cup game before the first leg and should be fresh. The return of Mateo Kovačić to the bench after several months out also adds welcome depth.

Real Madrid, meanwhile, are in fine form. While their season has blown hot and cold, the record UEFA Champions League winners have rediscovered form, winning four of their last games across all competitions.

Alvaro Arbeloa has a massive boost as Kylian Mbappe returned from a Knee injury, and Jude Bellingham from a hamstring problem, as per the BBC.

However, they’ll be without Rodrygo Goes (cruciate ligament, season over), Eder Militão (hamstring), Dani Ceballos (calf), and Alvaro Carreras (calf).

Head to Head

Both clubs have played five draws across 18 games since their first meeting in 2012. Six wins for City, seven for Madrid, averaging close to four goals per match.

City eliminated Madrid in the 2023 semi-finals (5-1 on aggregate), then the two clubs met again in the 2024 quarter-finals, producing a spectacular 3-3 at the Bernabéu, followed by a 1-1 at the Etihad.

Madrid advanced on penalties, in part due to a Bernardo Silva error. The following season (2024-25), Madrid snatched qualification through a devastating Mbappé hat-trick in the return leg.

This season, before the round of 16, the clubs had already crossed paths in the league phase last December. City won that match 2-1 in Madrid: Rodrygo opened the scoring, O’Reilly levelled after a Courtois error, and Haaland converted a penalty won after a foul by Rüdiger.

That evening, Los Blancos were without both Bellingham and Mbappé.

The first leg that was played last Wednesday was one for the history books. Valverde scored in the 20th, 27th and 42nd minutes, three finishes of three completely different kinds: a solo run finishing after a long ball from Courtois, a first-time left-footed strike across goal, and an acrobatic volley from a deft Brahim Díaz lob.

City, winners of the 2023 edition of the UCL, were not without quality in the first half, but Madrid’s counter-attacks were of surgical precision. Vinícius Júnior missed a penalty in the second half that would have made it 4-0.

Manchester City

City’s injury concerns are limited to two players: Joško Gvardiol (tibial fracture, long-term) and Rico Lewis (ankle). The remainder of the squad should be available.

Guardiola rotated against West Ham, which means Donnarumma, Rúben Dias, Rodri, Bernardo Silva and Marc Guéhi are well rested. The return of Kovačić to the bench is a welcome bonus for depth.

The Managers

Manager Pep Guardiola, 55, is enduring an uncommon campaign. His record against Real Madrid in the Champions League over the past five seasons reads: two progressions, three eliminations.

The criticism after the first leg at the Bernabéu centred largely on his decision to field three wide attackers around Haaland in a context where Madrid were quick on the counter. Guardiola defended the approach, pointing to City’s positive start to the match.

Tactical Overview

Arbeloa’s Madrid operate in a compact 4-4-2, defending in a deep block and breaking on the counter. Courtois, surprisingly, was adept at playing long over opposition pressing lines last time out, allowing Vinícius and Valverde exploit the space in behind while Brahim Díaz and Güler link the lines.

The problem for Madrid at the Etihad is that such a low defensive posture exposes them to sustained pressure. If City can impose their possession game, a makeshift Madrid defence will need to absorb 90 minutes of near-constant attack.

Madrid’s defence in the first leg wasn’t particularly superb, and an early goal for Pep’s side could open the floodgates.

City face a choice between two opposing approaches. The first is to open the game aggressively from the off, target an early goal and ride the emotional wave of the noise from the Etihad fans.

The second, more measured approach is to avoid conceding in the first half and allow pressure to build gradually.

The midfield battle will be decisive. Rodri against Tchouaméni and Valverde is a duel City lost convincingly at the Bernabéu. This time, City have home advantage, and Bernardo Silva will enjoy greater freedom to create.

The principal danger from Madrid will come through Vinícius on the break, while the availability of Trent Alexander-Arnold in build-up play gives Madrid the distribution quality to launch their attackers quickly.
